办理 Lasting Power of Attorney（持久授权书，及 Scotland 和 NI 的对应制度）
授权书让你在自己无法作决定时，由你信任的人代你作决定。Lasting Power of Attorney（LPA，持久授权书）只在 England 和 Wales 有效；Scotland 和 Northern Ireland 有各自的制度。

办理步骤
- England 和 Wales：决定你想办理健康和福利 LPA、财产和财务事务 LPA，还是两者都办。你必须年满 18 岁且具备心智能力。
- 选择你的受托人（attorneys），并决定他们共同行事还是各自行事。你可以指定替补人选。
- 在网上或纸质表格上填写，打印后由所有人在同一份原件上签名：你本人、受托人、见证人和一名 certificate provider（证明人）。
- 把 LP3 表格寄给你列为“person to notify”（须通知人）的人，然后向 Office of the Public Guardian（公共监护人办公室）登记 LPA。登记后才能使用。
- Scotland：办理 continuing（持续）和/或 welfare（福利）授权书，由律师或医生认证，并向 Office of the Public Guardian (Scotland) 登记。Northern Ireland：办理 enduring power of attorney（持久授权书），仅涵盖财务事务。
所需材料
- 受托人及替补人选的姓名和地址
- 证明人和见证人的信息（均须年满 18 岁）
- LPA 表格，打印后由所有人用墨水笔签名
常见错误
- 在复印件上签名或使用电子签名。所有人必须在同一份原件上签名。
- 让受托人为你的签名作见证或担任证明人。他们不可以。
- 以为 England 或 Wales 的 LPA 在 Scotland 或 Northern Ireland 有效，或适用于当地的房产。
